By documenting your wishes in advance, you ensure that your preferences are clearly understood while helping to ease the emotional and financial burden on those who matter most. Preplanning provides peace of mind, knowing that important decisions have already been made and your family will have guidance when they need it most.

A pre-paid arrangement is also a practical financial decision, allowing you to lock in today’s prices and protect against future cost increases. While discussing end-of-life arrangements can feel difficult, taking the time to plan ahead offers clarity, certainty, and comfort for everyone involved. Getting Started with Pre-Planning

  1. Provide Your Information
    We will collect the information required to establish your pre-planning file and accurately document your instructions.
  2. Select Your Arrangement
    Choose the arrangement that best meets your needs, including Direct Cremation, Direct Burial, or Transfer of Body Donation to University of Western Ontario.
  3. Select Your Payment Option
    Choose whether you would like to prepay your arrangements. All funds are held in trust using Guaranteed Funeral Deposits (GFD), or set up a not paid file with payment at the time of need.

Information Needed to Complete Your Pre-Arrangement

Information We’ll Need

To complete a pre-arrangement, we gather the same information that will later be required for legal documents, such as registering the death.

To register a death at the time of need Thames Valley Cremation Services Inc. will submit two documents to London City Hall.

The first document is a Statement of Death (Form 15): a family member and a funeral director will complete this form together. It includes personal information about the deceased (e.g., family history, date of birth, place of birth, date of death, occupation, social insurance number, etc.). The second document that is registered at City Hall is a Medical Certificate of Death (Form 16), which is provided by the doctor who pronounces the death. This information is forwarded to the Office of the Registrar General in Thunder Bay, Ontario. City Hall will provide Thames Valley Cremation Services Inc. with a Burial Permit (Form 19)

A Burial Permit is required before any direct cremation, direct burial or body donation can take place.

Having this information on file ensures that, when the time comes, everything is already in place and ready to move forward efficiently and without delay. Your next of kin or executor won’t need to search for records or guess at details, helping to reduce stress and the amount of work for them during a difficult time. 

What is a not paid file? 

Not-Paid File
 
You may choose to set up a Not-Paid File, where we record your wishes and keep them safe for the future. The file is set up exactly like a prepaid file; however, no payment is taken. Payment is simply made at the time of need.

Pre-Arrangements with Payment (Direct Cremation)

This begins with an appointment with us at our office or your residence. During this time, we gather the required information. We complete a Thames Valley Cremation Inc. “Pre-Need Contract Funded by Trust”, which outlines all the services, supplies, disbursements, and HST. We take care of all paperwork and documentation and set up your prepaid file, including the required trust form from GFD—the trust company that holds your funds securely. We take pride in being exceptionally organized and detail-oriented, ensuring every step is handled with accuracy, care, and professionalism.

The total amount invested with GFD for Thames Valley Cremation Services Inc. is $2,054.80 (Including HST). 

An additional contract is needed to pre-pay the cremation with the crematorium in the amount of $640.20 (HST included).

Thames Valley Cremation Services Inc. will complete all the paperwork and will submit your payment to Crematorium - Woodland. They will process the payment, and will issue a receipt to the purchaser of the contract and invest the funds into the trust with Guaranteed Funeral Deposits (GFD).

Cremation - Woodland $640.20
Grand Total $2,695.00

Funds are held in the purchaser’s name for the benefit of the beneficiary, who may be the same person or someone else. When the pre-need contract is paid in full, the contract becomes guaranteed.

The total payments made, along with any interest or growth earned, will be applied to the future cost of the services and supplies. If payments plus interest are insufficient to cover future prices, Thames Valley Cremation Services Inc. will absorb the difference. If the trust funds exceed the total cost, the surplus will be refunded to the estate or designated beneficiary within 30 days after the death has occurred. 

Funeral Arrangements Wallet Card
For those who choose to establish a Not-paid file, or who complete a pre-arrangement with payment funded by trust, we provide a Funeral Arrangements Wallet Card confirming that direct cremation or direct burial arrangements are on file with Thames Valley Cremation Services Inc.

It's a simple card that can be helpful at the time of need, as it clearly identifies who to contact.

Are There Payment Options on the $2,054.80?

Yes, we offer monthly payments. 

Payments can be scheduled for the 1st or 15th of the month and are made directly to GFD. Two payment methods are available:
• Automatic bank withdrawal
• Visa or MasterCard

Transferring an Existing Pre-Paid Arrangement

If you already have pre-paid arrangements with another funeral home, you can transfer them to Thames Valley Cremation Services Inc. Pre-paid funds remain your property until the time of need, and any excess funds after services are provided are returned to the estate within 30 days after the death has occurred.

Making Things Easier for Your Family

This will save a lot of time and stress for your family. Even if you prepaid your direct cremation, under the law it does not authorize the cremation. However, all they will have to do is call us at the time of need, make an appointment, and sign documents because you have supplied all the information and payment ahead of time.

Once Guaranteed Funeral Deposits (GFD) receives your payment, they will issue the purchaser a Trust Certificate
