Service Canada helps manage many federal benefits including CPP and Old Age Security.
During the at need meeting we will complete and fax Service Canada the Notification of Death Form to Canada Pension Plan and Old Age Security. The purpose of this form is to notify Service Canada of death a Canada Pension Plan (CPP) and /or Old Age Security (OAS) beneficiary to stop the payment of a CPP and / or OAS beneficiary to stop the payment of CPP and /or OAS benefit in order to avoid overpayment.
Some families may quality for the Canada Pension Plan Death Benefit, which is a one-time payment to the estate or eligible individual. The maximum CPP Death Benefit is $2,500.

Thames Valley Cremation Services Inc. will complete this form for you and all the necessary documents that are required to go with the form. All you have to do is mail it in or take it to Service Canada.
Surviving spouses or dependent children may be eligible for ongoing CPP survivor benefits.
The survivor's pension can be applied for by a person who is legally married or in a common law relationship with a person who has died. May also apply to children of the deceased who are under the age of 18 or between the ages of 18 and 25 attending school, college or university full-time.

If the deceased person held a valid Canadian passport, it should be returned to the Government of Canada so the passport can be cancelled.

If the passport has already expired, it does not need to be returned. However, if the executor/next of kin would like the Government of Canada to securely destroy the expired passport, they may mail it with a copy of the death certificate and a written request for its destruction.
