Please complete the Body Bequeathal Form in advance. While Western University does not accept or retain this form prior to death, Thames Valley Cremation Services Inc., will securely keep a copy on file for your records. At the time of death, the completed documentation will be provided to the Western University, and acceptance into the program will be determined by the Department of Anatomy & Cell Biology.
One of the most unique and meaningful parts of Form B is the reflection page.
This page invites families to share personal details about their loved one, beyond medical information, offering insight into who there were as a person. You may choose to include interest, hobbies, life experience, etc. These details help students understand and appreciate the donors as individuals.
If donation is something you are considering, you are welcome to take the time to prepare this section in advance. Thames Valley Cremation Services Inc., will keep this information on file and guide your family through the next steps when the time comes, including submitting Form A & B

Professional consultation and a scheduled appointment with a licensed funeral director. It covers the co-ordination of all required activities before, during and after the body bequeathal process, including services and supplies provided by Thames Valley Cremation Services Inc., as well as those arranged through third-party providers.

Completion and filing of all documents necessary to carry out the services and supplies requested, including, but not
limited to death registration, burial permit, coroner’s cremation certificate and cremation application. We also provide
15 Proof of Death Certificates initially for estate settlement purposes.

Providing secure shelter in a licensed holding facility

Professional services of a licensed funeral director to complete all required visible screening of the deceased, including answering questions and providing necessary information to the receiving institution (Western University – Body Bequeathal Program) for the purpose of determining eligibility and acceptance. Acceptance into the program is determined solely by the receiving institution.

Once all the necessary documents have been obtained, and permissions from the Western University and the executor/next kin or administrator are completed, we will transfer the deceased to Western University.
Once the deceased has been transferred to the Western University, Thames Valley Cremation Services Inc.’s services are complete, except for the following:

The kilometer allowance is shorter on the body bequeathal package than on direct cremation
package. The direct cremation package is 50 km, or the designated service area as stated on our
direct cremation package price list or website.
If you are considering this option as final disposition, timing is essential.
To avoid problems with embalming requirements at Western University, it is necessary that the
University receive the remains within 48 hours of the time of death.
