Form A

Please complete the Body Bequeathal Form in advance. While Western University does not accept or retain this form prior to death, Thames Valley Cremation Services Inc., will securely keep a copy on file for your records. At the time of death, the completed documentation will be provided to the Western University, and acceptance into the program will be determined by the Department of Anatomy & Cell Biology.


Form B

One of the most unique and meaningful parts of Form B is the reflection page.
This page invites families to share personal details about their loved one, beyond medical information, offering insight into who there were as a person. You may choose to include interest, hobbies, life experience, etc. These details help students understand and appreciate the donors as individuals.

If donation is something you are considering, you are welcome to take the time to prepare this section in advance. Thames Valley Cremation Services Inc., will keep this information on file and guide your family through the next steps when the time comes, including submitting Form A & B

Thames Valley Cremation Services Inc. is a local transfer service operator that can assist with body donation to Western University.

If you are considering Body Donation as an end-of life option, Thames Valley Cremation Services Inc. can help arrange for a transfer of your body to Western University. Pre-arranging and pre-paying is strongly recommended, as the University's requirements for timing of receipt of a body donation can be strict. It is necessary that the University receive the remains within 48 hours of the time of death.

Donors should be aware that while most bodies that are bequeathed to the program are accepted, there is no guarantee. There are acceptance criteria that the donor must meet in order to be eligible. Individuals who may be considering body donation are also encouraged to make alternative disposition arrangements, such as cremation, in the event that their body is not accepted by Western University. Body Donation Package

Services

Service & Staff Services – Consultation & Co-ordination of Services

$344.00

Professional consultation and a scheduled appointment with a licensed funeral director. It covers the co-ordination of all required activities before, during and after the body bequeathal process, including services and supplies provided by Thames Valley Cremation Services Inc., as well as those arranged through third-party providers.


Documentation – Permits, Forms, etc.

$350.00

Completion and filing all documents necessary to carry out the request services including but not limited to death registration at city hall, burial permit, and required documentation for body bequeathal. We also provide 15 Proofs of Death Certificates initially for estate settlement purposes.


Transfer of Deceased – Additional mileage fees apply beyond 40 km for Body Donation.

This service includes the transfer and vehicle from the place of death directly into our care to our holding room. The
deceased is not transferred directly from the place of death to Western University campus. Includes up to 2 staff.


Facilities for Shelter:

Providing secure shelter in a licensed holding facility


Assessment & Co-ordination with Receiving Institution

$195.00

Professional services of a licensed funeral director to complete all required visible screening of the deceased,
including answering questions and providing necessary information to the receiving institution (Western University
– Body Bequeathal Program) for the purpose of determining eligibility and acceptance. Acceptance into the program
is determined solely by the receiving institution. (Western University – Body Bequeathal Program) for the purpose of

The transfer of the body for donation purposes needs to be expedited within 48 hours of death.


Vehicle to Transfer the deceased to Medical Science Building on Western University Campus:

$156.09

Once all the documents have been signed by the executor, next of kin or administrator and the death has been
registered with city hall and permission has been granted from Western University. Thames Valley Cremation
Services Inc. will transfer the deceased to the designed area on campus.

Once the deceased has been transferred to the Western University, Thames Valley
Cremation Services Inc.’s services are complete, except for the following:

Fee set and charged by the local municipality to register a death that took place in another
municipality. If death occurs outside of the City of London


Obituary for Website and Facebook Page

No Charge

An online obituary is a digital tribute to the deceased, published on our website, rather than in a
newspaper. It serves to share the deceased’s life story, memorial details, and condolences with
family and friends.


Additional Mileage

The cost for transporting a deceased person on the initial transfer beyond 40 km is $3.00km

The kilometer allowance is shorter on the body bequeathal package than on direct cremation package. The direct cremation package is 50 km, or the designated service area as stated on our direct cremation package price list or website.

If you are considering this option as final disposition, timing is essential.

Pre-arranging and pre-paying the Transfer for Body Donation is highly recommended. This process is time-sensitive, and having everything in place helps avoid delays. It reduces the steps for your family; they already know what to expect and help ease their stress, as all necessary information such as your social insurance number, date of birth, place of birth and other details have already been provided to us.
